Job Description & Experience:
We are seeking SERVICE COORDINATORS/CASE MANAGERS for the following boroughs: Manhattan, Brooklyn and Bronx
Early Intervention Service Coordination (EISC) is a service division of Public Health Solutions of New York City, Inc.
Early Intervention Service Coordination EISC/Public Health Solutions is contracted with the New York City Early Intervention Program (NYCEIP) to provide service coordination throughout the five boroughs for infants and toddlers with known or suspected developmental delays.
We are seeking motivated individuals who have a strong commitment to helping families.
Responsibilities include providing case management to families with infants and toddlers with developmental delays; developing plans and strategies to meet the family''s needs, securing a multidisciplinary evaluation to determine the child''s eligibility; and participating in the development, implementation, and monitoring of the Individualized Family Service Plan (IFSP).

- Bachelor's Degree in Health or Human Services, Psychology (or Forensic Psychology), Sociology or Social Work, OR Bachelor's Degree in fields other than those specified, and at least 2 years+ experience in case management with children and families.
